The Licensed Practical Nurse (LPN), is a specifically trained member of the Fort Smith Clinic team. Licensed Practical Nurse is to promote wellness, improve outcomes, and self-care within a defined population of patients in an Outpatient Clinic. The LPN provides direct patient care to assigned patients. As a professional nurse, the nursing process is utilized in the provision of care. The LPN practices within the limits of the Nurse Practice Act of the State of Arkansas and adheres to institutional policies, procedures and standards.

The LPN works under supervision to best serve the needs of the patients and Family Medical Center. The LPN will provide patients support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state and will work with the individual patient, their families, and providers of care to develop and implement individual health plans, and provide support to the patient in self-management. LPN will utilize the Care Manager Dashboard to perform previsit planning before each patients visit, provide information during huddles to clinical staff and physician before the patient visit in order to assist in closing care gaps, return patient phone calls and perform phone triage, refill medications as directed by physician, assist with referrals, obtaining of durable medical equipment, and correspondence received via fax, email, postal service, and/or patient delivery. Assesses incoming phone calls and assist front office staff with walk-in patient issues. The position may at times be asked to fill in for absent nurses or other clinic personnel, and/or perform assigned administrative or other patient care duties.
This position must be able to perform all duties of the Licensed Practical Nurse II and performs other duties to support the patient care activities as needed.Qualifications
Required: Graduate of an accredited Nursing Program is required. Valid LPN License, minimum of 1 year of LPN experience; excellent communication skills, strong work ethics and accountability, basic computer skills.
Preferred: 2 years or more nursing experience, current BLS
